First Visit

• Fill in the form "Diagnosis Application" at table, and put at the window two (image 1). You will be called by rotation. Please present the insurance certificate. If not, you have to bear the full expenses.
• You will receive a file with registration card and clinic record label in it at window two. Please submit it to the department where you receive treatment. If you have a referral form, please submit it with the file.
• You can use the registration card for every department in the University Hospital.
  

Patients without a referral

•  As a matter of principle, we only accept patients who have a referral form.  A patient without a referral can be seen in a general practice, but in this case the patient is charged 7,700 yen for the cost of a first consultation.
